CACH Net Achievement Award

The CACH Net Annual Achievement Award was created in 2024 to recognize Canadian CACH Net members who have made outstanding contributions in their career within the adult congenital cardiology field.

Eligibility Criteria

CACH Net members are asked to nominate an individual:

  1. Whose work has significantly aided the growth of and support to the adult congenital cardiology field.
  2. Is a member of CACH Net in good standing; exceptions can be considered at the discretion of the CACH-Net Board in exceptional circumstances i.e., retirement.
  3. Has highly respected attributes relative to this award.

Selection Criteria

A nomination submission must include the following documents:

  1. A Letter of Recommendation summarizing the candidate’s highly respected attributes relative to this award; and
  2. A 1 to 2-page biography of the nominee.

The deadline for submitting nominations is Wednesday, May 1, 2024 (Noon EDT). Submit to the CACH Net Review Committee at cachnet@ccs.ca.

Award Prize

The recipient will receive a crystal award. The award will be presented at the Canadian Cardiovascular Congress (CCC) at the CACH Net Annual General Meeting (AGM).
